- Material Safety Data Sheet/Safety Data Sheet (ISO or GHS format)
- DENR Identification Number as Hazardous Waste Generator
- Environmental Compliance Certificate
- Discharge Permit
- Permit to Operate for APCD and/or APSI
- Summary of Importation Data (for importers)
- Chemical Management Plan (includes storage, handling, disposal, transport, Safety and health requirements and environmental controls at the workplace).
- Management Operation Flow Chart (Process flow on the usage of the chemical)
- Contingency/Emergency Plan (includes response procedure on spills, contact, inhalation, ingestion, transport, fire, etc.)
- List of Users/Customers with corresponding projected/required volume. (for importer/distributor).
- Groundwater/Surface Water Monitoring Results (for user/manufacturer)
- Self Monitoring Report (for user/manufacturers)
- Photos of the storage facility/warehouse (photo showing the chemical container and chemical labeling, view of the interior of the storage area, safety equipments, if any).
- Basic knowledge posed by the use and release of the chemical (e.g. photocopy of training certificate of PCO related to Chemicals, Safety, Environment and/or training attendance of employees relating to chemical management, safety and environment. Include the number of employees handling the chemical.

- Pictures (8”x10” in size), Detail and Location plans of magazine properly labeled.
- Twelve (12) months consumption report attested by PD/CD for Prov’l Applicant & duly notarized for NCR Applicants.
- Latest Monthly Report for explosives duly endorsed or attested by PD/CD (for Prov’l Applicant) and duly notarized (for NCR Applicant).
- Endorsement from ROPD & PD/CD (for Prov’l Applicant).
- NSW Application Form.
Additional Requirements of the Philippine Bomb Data Center (PBDC)
- Detailed Monthly Consumption for the last twelve (12) months providing information on the names of person/entities, their respective CSG License to Purchase Number (Permit), address/es and volume/amount of explosives/explosive ingredients purchased.
- Material Safety Data Sheet (MSDS) of each Explosive Products

Procedures

Procedure for securing Chemical Control Order Importation Clearance for Asbestos & asbestos containing products, Cyanide & cyanide compounds, Mercury & mercury compounds, Lead & lead compounds and Polychloinated Biphenys under Chemical Control Order (CCO)
Every importation of chemicals covered under the Chemical Control Order (CCO) is required to secure Importation Clearance to the DENR-EMB Regional Offices.
- Applicant files application with attached requirements to the Pollution Control Division (PCD)
- PCD screens and checks documents. If complete, Cashier Section receives payment and issues receipt and then forwards to Records Section
- Records Section receives document and forwards to PCD
- PCD evaluates document, recommends additional requirements and the issuance of Importation Clearance and endorses to the Office of the Regional Director
- Office of the Regional Director approves/disapproves the CCO Importation Clearance
- Records Section releases the Importation Clearance.
