  1. Letter Request (State purpose).
  2. Board of Director’s Resolution. (Secretary’s Certificate)
  3. Comprehensive Company Profile.
  4. PNP Form No. 6 duly accomplished.
  5. Original Special Bank Receipt (LBP)
  6. Php 6,000.00
  7. Photocopy of license.
  8. Inspection Report/Security Survey attested by PD (Prov’l Applicant) and noted by I&E, FEO (NCR Applicant).
  9. Pictures (8”x10” in size), Detail and Location plans of magazine properly labeled.
  10. Twelve (12) months consumption report attested by PD/CD for Prov’l Applicant & duly notarized for NCR Applicants.
  11. Latest Monthly Report for explosives duly endorsed or attested by PD/CD (for Prov’l Applicant) and duly notarized (for NCR Applicant).

  1. Detailed Monthly Consumption for the last twelve (12) months providing information on the names of person/entities, their respective CSG License to Purchase Number (Permit), address/es and volume/amount of explosives/explosive ingredients purchased.
  2. Material Safety Data Sheet (MSDS) of each Explosive Products

  1. Receiving Clerk checks the completeness of submitted requirements
  2. Processor process application and endorsement of documents
  3. Proof Reader checks the prepared documents
  4. Chief Clerk checks and reviews the authenticity of submitted requirements.
  5. Chief, Permits and Other Licenses Section reviews and endorses application to Chief, Firearms and Licensing Division
  6. For concurrence of Chief, Firearms and Licensing Division
  7. For concurrence of Assistant Chief, Firearms and Explosives Office
  8. For concurrence of Chief, Firearms and Explosives Office and transmittal of application to CSG MSG Center
  9. For review and concurrence of CDS, CSG and transmittal to DD, CSG

  11. For recommendation for approval of D, CSG

Philippine Nuclear Research Institute

Issuance of Certificate of Release

  1. Receives request for the issuance of Certificate of Release.
  2. Reviews and evaluates the submitted documents, i.e. airway bill and shipping documents, also, refer to radioactive material license for the coverage of authorization.
  3. Pays corresponding fees at the PNRI Cashiers' office.
  4. Prepares the Certificate of Release.
  5. Reviews and verifies the prepared Certificate of Release.
  6. Signs and Approves the Certificate of Release for release to the license.
  7. Affix the PNRI dry seal and release certificate.
