    Clearance for Transportation of Biological Specimen

    CHECKLIST OF REQUIREMENTS

    A. For Clinical Research Study

    I. For Import/Export/Domestic

    1. Request Letter addressed to the following:
      1. Bureau Director for NCR Station/BOQ Main Office
      2. BOQ Station Chief for BOQ Provincial Stations: Tabaco, Palawan, Laoag, Bacolod, Iloilo, Kalibo, Cebu, Davao, Tacloban, Cagayan de Oro, Zamboanga
    2. with contents as follows:
      1. Type of Specimen and net quantity per package
      2. Name and address of facility where specimen is collected
      3. Name and address of facility where specimen is to be transported
      4. Purpose of the specimen
      5. Declaration that specimen is infectious or non-infectious
    3. FDA Approval for the study (Food and Drug Administration)
    4. Accomplished application form (Annex A or C)

    5. Document stating that the shipper, handler or courier are IATA trained and abided to its rules and regulations regarding handling and packaging
    6. Airway Bill (for Import only)
    7. Other supporting documents (as deemed necessary)

    B. For Other Purposes except Clinical Research Study (Import/Export/Domestic Transportation)

    I. For Import/Export/Domestic

    1. Request Letter Request Letter addressed to the following:
      1. Bureau Director for NCR Station/BOQ Main Office
      2. BOQ Station Chief for BOQ Provincial Stations: Tabaco, Palawan, Laoag, Bacolod, Iloilo, Kalibo, Cebu, Davao, Tacloban, Cagayan de Oro, Zamboanga
    2. with contents as follows:
      1. Type of Specimen and net quantity per package
      2. Name and address of facility where specimen is collected
      3. Name and address of facility where specimen is to be transported
      4. Purpose of the specimen

      PROCEDURES

    1. Submit the complete required documents.
    2. Proceed to the cashier for payment of the corresponding fee:
    3. FEES
        For Clinical Research – Php 500.00
        For Diagnostic and other purpose except Clinical research – Php 500.00
    4. Give the official receipt to the Quarantine Personnel.
    5. For Quarantine clearance date of releasing is on the day of application:

    Sign the receiving copy and receive the original copy of the Quarantine clearance
