A.2. Schedule “B” - List of Manufacturing Material for Optical Media

Any material, such as, but not limited to, optical grade polycarbonate or polycarbonate substitutes with physical properties suitable for the manufacture of optical media. Suitability shall be measured by a melt flow index (MFI) which shall be determined by the OMB. Unless otherwise determine, polycarbonate or polycarbonate substitutes with MFI of not less than 45 grams per ten (10) minutes at 300 degrees Celsius at 1.2 kilogram load shall be considered as suitable as manufacturing material, for purposes of these Rules.

Registration/Licensing of Optical Media Business

  1. 1. Secure OMB Application Form and list of requirements from the frontline receiving/assessing area (for walk-in applicants) or download them from the URL: www.omb.gov.ph
    -- If website are down, manual application form are available.
  2. 2. Secure clearance of NO Admin Case from Legal Division (LD)

  3. 3. Submit LD clearance to Licensing Receiving/ Assessing Clerk together with the duly filled-up and notarized application forms with complete requirements for checking and assessing of corresponding licensing fees.

  4. 4. Pay the required fees by presenting the Assessment Slip(s) and cash and/or check to the Cashier.

  5. 5. Present the Official Receipt and return the Assessment Slip to the Licensing Receiving/ Assessing Clerk for the Issuance of OMB application form (duplicate) as Applicant’s copy.
  6. 6. Claim processed License from the Licensing Releasing Clerk after five (5) working days from the date of application. Present authorization letter with photocopy of valid ID if the person claiming is not the applicant.

Import/Export

  1. 1. Secure and fill up Application Forms for Import and/or Export Permit

    FOR REGULAR CLIENTS (Prior to application of Import/export Permit, Licensing must done first.):

    1. a. Fill-up application form online through the Philippine National Single Window URL: www.nsw.gov.ph
    2. b. Print form with the corresponding bar code number.
    3. c. Have it notarized and submit forms with complete requirements to the Registry Receiving /Assessing Officer for checking and assessment of fees.
    4. -- If website are down, manual application form are available.

    FOR NEW AND ONE-TIME CLIENTS:

    1. a. Secure OMB Application Form for Import and/or Export Permit and list of requirements from the front line receiving/assessing area. Forms may also be downloaded from http://omb.gov.ph/ombnew/index.php?page=services§ion=import
    2. b. Submit the duly filled-up and notarized application form with complete requirements to the Registry Receiving/Assessing Officer for checking and assessing of fees.

  2. 2. Pay the required fees by presenting the Assessment Slip and cash and/or check payment to the Cashier.
