Documentary Requirements Procedure Lead Time Processing Fee

General Requirements for Licensing, Registration and Accreditation as Meat Importer:

REGULAR MEAT IMPORTER

A. Requirements:

  1. 1. Letter of Intent addressed to the NMIS Executive Director
  2. 2. Notarized Accomplished Application Form with 2x2 coloured picture
  3. 3. Mayor’s Permit, Sanitary Permit, Barangay Clearance (certified true copy)
  4. 4. Bureau of Custom - Interim Customs Accreditation Registration (iCARE) (certified true copy)
  5. 5. For Single Proprietorship:
    • DTI Registration (certified true copy)
    • Certificate of Capitalization from a refutable Bank of at least Php5,000,000.00 (original copy)

    For Corporation/Partnership/Cooperative:

    • Proof of SEC Registration (certified true copy)
    • Certificate of Good Standing for the current year (original copy)
    • Latest General Information Sheet and certified list of incumbent executive officers of good standing from the SEC or Cooperative Development Authority (certified true copy)
    • Articles of Incorporation with provisions on the corporation’s capital of at least Php5,000,000.00 (certified true copy)
    • Treasurer’s Affidavit certifying the receipt of payment for the subscriptions of the incorporators (original copy)
  6. 6. TIN Certificate of Registration (certified true copy)
  7. 7. Audited Annual Financial Statement for the previous years (certified true copy)

    The following shall be exempted:

    • Newly established business which is less than one year in its operation
    • Those granted tax incentive by the Bureau of Investment (BOI)
    • Other Entities or bodies which are explicitly exempted under the law, agreement or other similar legal instruments
  8. 8. Notarized Lease of Contract or Proof of Ownership of Cold Storage or Dry Warehouse Facility (certified true copy)
  9. 9. Accreditation Certificate of all Cold Storage Warehouses – public commercial and in-house CSWs (certified true copy)
  10. 10. Notarized Affidavit of Undertaking of conditions for accreditation (original copy)
  11. 11. Schedule of Fees
    1. a. Accreditation Fee – Php 4,000.00
    2. b. Mailing Fee – This shall be based on the current/prevailing market price
  12. 12. Certificate of Attendance to the Monthly Meat Importers Orientation on NMIS meat importation procedures and other relevant policies by the owner/manager/broker at the NMIS Office

B. Additional Requirements:

  1. a. For Meat Processor
    • Meat Processing Plant (MPP) Accreditation Certificate (certified true copy)
    • Rated Capacity of the MPP issued by the DA Inter-Agency Committee (certified true copy)
    • HACCP Certificate of Products utilizing Indian buffalo meat (certified true copy)
  2. b. For Meat Trader
    • List of Clients and address for the past year
    • Target Clients and address for the current year
  3. c. For Institutional User
    • Department of Tourism (DOT) Accreditation/Classification (certified true copy)

Note:

  1. 1. A company’s/cooperatives authorized representative shall submit a Board Resolution of the Corporation/Cooperative Authorizing the Legal Representative together with a copy of two valid IDs with picture and signature.
  2. 2. A Broker shall submit a SPA, certified true copy of the License as Customs Broker, and copy of two valid IDs with picture and signature
  3. 3. All documents must be secured in a folder according to the checklist requirements.

  12. 12. Proceed with 100% inspection with BOC (break seal for stripping)
  13. Alternate Process: Proceed with 100% inspection after 24 hours (without BOC)
  14. 13. Release of VQMILC (BOC and Importer's Copy; Photocopy of used VQMILC to be returned to VQO by licensed broker)

PROCESSES (ACTIVITY)

  1. a. Submission of complete requirements by importer and documentary assessment by Regional Technical Operations Center-Meat Import and Export Section (RTOC-MIES)
  2. b. Conduct of onsite inspection by the RTOC-MIES and NMIS-Meat Import and Export Division (NMIS-MIED)
  3. c. Endorsement by RTOC-MIES to Office of the Executive Director up to the time the COA is approved
  4. d. Release of COA by NMIS Records Section
