  1. 1. Duly accomplished application form.
  2. 2. Export permit/certification of origin from exporting country.
  3. 3. For live specimens, veterinary.phytosanitary certificate issued by the authorized government agency of the country of origin.
  4. 4. Payment of fee.
  5. Special Instructions
    The following should be indicated in the Import Permit under box no. 6 "Special Conditions":

    The Veterinary Quarantine Clearance must be obtained from the Bureau of Animal Industry prior to departure from the country of origin.(for live speciments only).

    The purpose of importation should likewise be indicated under this box, e.g. "for breeding purposes only","for commercial trade purposes", or "for pet purposes", among other purposes.
